Niagara Cerebral Palsy- Niagara Falls, NY $ 2,500.00
Purpose: The funds will be used to purchase educational materials related to a dental education programs for preschool aged children, parents and guardians. This is a pilot program for children with intellectual and developmental disabilities.

Church Health Services- Beaver Dam, WI $ 2,000.00
Purpose: This program will target 15 Amish Community Schools, which include 600 students and their parents. Funding will be used to provide educational materials and dental hygiene kits. $ 3,000.00 requested
McMillian Center for Health Education- Fort Wayne, IN $ 2,647.00
Purpose: Support the ongoing Brush! program which available to children birth to age 5 and their parents. This will provide the Brush! program to 250 preschool aged children.

Alliance of the Stark County Dental Society – Canton, OH $353.00
Purpose: To support Healthy Smiles from the Start program at the Affinity Medical Center and Alliance Community Hospital as part of the prenatal and postnatal education classes for new mothers and caregivers. The purpose of the grant is to provide oral health care supplies. $ 353.00 requested
Salt Lake Donated Dental Services – Salt Lake City, UT $ 1,500.00
Purpose: The donation will allow SLDDS to teach an estimated five oral hygiene classes to 225 low-income families and provide dental kits. $ 2,000.00
AADA presented by Laurie Osborn, President - $ 5,000.00
Purpose: Conference 2015 DHE Keynote Speaker in Memphis TN. Expenses related to support the speaker Stan Brock, founder of Remote Area Medical, and staff who will be present at the meeting, booth, and Lunch and Learn at the University of Tennessee Dental School. He will speak to attendees and spouses regarding access to care in our country and abroad. Each attendee will receive a copy of the speaker’s book. $ 5,000.00 requested
